Varying consultancy options to identify and analyze failure-factors and causes

Practicing failure investigation and root cause analysis helps to solve problems by identifying and correcting root causes and failures of events rather than just addressing their symptoms.

Root cause analysis is an iterative process and a tool of continuous improvement, which can be applied in all cases of nonconformity or recognized improvement potential. It prevents re-occurrence of the same issue or problem so that there is no need for unnecessary shut-downs, repairs or unplanned expenditure.

The following options are available:

  • Physical – a failure investigation only. This identifies the physical mechanism, and the likely initiating event for the failure. This is a purely scientific investigation; the report will provide facts relevant to the evidence collected and will not make any assumptions as to organizational factors or human influences related to the failure.
  • Causal – a physical failure investigation with an analysis of the factors contributing to the event. This approach will ascertain the facts that led to the failure and identify corrective technical actions. The human and organizational causes will not be investigated.
  • Full RCA – a physical failure investigation backed up by a full analysis of the human and organizational factors contributing to the failure. This type of investigation offers the most valuable learning, and the highest level of transparency. The effort required by our clients to help us complete full RCA's must not be underestimated.

First, we examine organizations' state of readiness, which includes spares philosophy, warehousing philosophy and drills (participants, nature, frequency, debriefing). Additionally, we offer consultancy in terms of:
